The woman, who had just returned from taking her dog for a walk, said the man - Mr Mazs - fell flat on his back and as he did so his "head fell back on the concrete very hard".
She said: "It was hard enough to hear it hit the floor.
"You would assume it would sound like a crack but it didn't - it was more of a hard thud".
She said Mr Mazs landed on the floor in a "starfish position with his arms and legs wide open".
The woman said she did not see Mr Mazs move but saw Montgomery, who she recognised from him living in the flats, with his partner near where the man fell.
She said Montgomery was shouting and swearing, telling the felled Mr Mazs and his friend to go.
She said he was telling them to "f**k off, there's kids here, you shouldn't be doing drugs here".
Montgomery also threatened to "kick him".
The woman said Montgomery, who apologised to her for swearing, seemed "angry, very angry".
She said a friend of Mr Mazs managed to sit him up after about five minutes but he was still unconscious and she was able to see "a lot of blood".
When Mr Mazs, who was still unconscious, was helped away by two friends, the woman said Montgomery was shouting "don't f***ing come back".
The trial continues.
